Job Title: Case Manager

Location: Abuja

Job Type: Full Time

Duration: One year

Job Role:

The Case Manager ensures that his/her assigned clients receive a coordinated range of client-centered services linking clients with health care, psychosocial and other services. The case manager is responsible for completing biannual reassessments with clients to include completion of a care plan in addition to seeing clients periodically to address their current needs and checking compliance to medical care and adherence HIV medications. The case manager is responsible for entering all client data, units of service, and case notes into the database in a timely manner. The case manager reports to M&E Officer and Project Manager.

 

Essential Duties and Responsibilities

  • Enroll households according to target streams into CFHI’s IHVN ACHIEVE OVC program within allocated communities in FCT.
  • Service delivery to enrolled beneficiaries.
  • Work closely with community members to strengthen knowledge and promote awareness on HIV/AIDS prevention through behavioural change.
  • Responsibility for own clients/cases assigned by supervisor.
  • Assist and encourage clients to access medical care.
  • Data entry and update into NOMIS on a timely basis
  • Maintain assigned clients’ medical and service record files and be responsible for files completeness.
  • Conduct initial comprehensive assessment of clients’ needs and support systems.
  • Develop a comprehensive, individualized service plan and coordination of the services required for implementation of plans for clients.
  • Monitor to assess the efficacy of the plan and periodic re-evaluation and revision of the plan as necessary.

 

Others

  • Participate in CFHI / IHVN ACHIEVE project mandatory trainings
  • Provide case management services to own panel of clients including:
  • Assist clients in accessing routine medical care for HIV.
  • Refer appropriately to other AIDS and social service agencies.
  • Provide HIV risk reduction education and prevention
  • Ensure client’s compliance with medications, education as necessary
  • Perform other duties as assigned.
  • Attend mandatory monthly review meetings and send periodic reports

 

Qualifications/ Requirements

  • Minimum of OND certificate in any discipline.
  • 1-2 years of experience in OVC case management.
  • Basic knowledge of computer software such as Microsoft Office is mandatory.
